Application Forms for Licensing Act 2003

Application forms are now available to download in two different formats.

Guidelines on how to Fill in the Forms

  • application/pdfPDF Application forms - Application forms in PDF format should be printed and completed in block capitals using black ink.
  • application/mswordWORD Application forms - Application forms in Word document format are protected documents that can be completed (filled in) on-line by double clicking in the chosen area and typing as normal and then should be printed for signature and submission to the Licensing Authority.
  • List of Application forms - A list of all the Licensing Application forms in both formats we currently have on the website.

All the forms, guidance and any further information concerning an application can be found by clicking on the relevant licence heading.

Completed forms should be signed, dated and posted to The Licensing Section, Environmental Health Service, Stroud District Council, Council Offices, Ebley Mill, Westward Road, Stroud, Glos, GL5 4UB.

All applications must be accompanied by the relevant fee and you may wish to keep a copy of the completed form for your records.

Applicants may find it advantageous to read the guidance notes at the end of the form or download the relevant guidance notes before completing the forms.
